Insurance & payment information
Out-of-Network Insurance Accepted
Dr. Naqvi is an out-of-network provider. Many PPO insurance plans offer out-of-network benefits that reimburse a portion of your physical therapy costs.
- You pay Dr. Naqvi directly at time of service
- We provide a superbill (detailed receipt) after each visit
- You submit the superbill to your insurance for reimbursement
- Reimbursement typically ranges from 40–80% depending on your plan
Cash / Self-Pay Accepted
No insurance? No problem. Self-pay rates are straightforward with no hidden fees or surprise charges.
- All rates are listed above — what you see is what you pay
- Payment collected at time of service
- Accepts cash, credit/debit card, Venmo, Zelle
- No medical biller or third-party billing involved
Why out-of-network?
In-network insurance reimbursement rates often force clinics to see 3–4 patients per hour to stay profitable. By operating out-of-network, Dr. Naqvi can dedicate a full hour of 1-on-1 care to every patient — which means faster results, fewer total visits, and better outcomes. Many patients find they actually spend less overall compared to longer in-network treatment plans.
No referral needed
California is a direct access state — you can see a physical therapist without a doctor's referral. However, some out-of-network insurance plans may require a referral for reimbursement. Check with your insurance provider if you plan to submit for reimbursement.
